Their framework changed the way I design every workshop I run, and it will change the way you look at your team. If you are an HR leader who has spent a significant budget on training programmes that produced mediocre results, or a manager watching your team repeat errors they were trained to avoid, read every word of what follows. The diagnosis is sharper than anything a competency framework will show you. The Philosophers Who Mapped Professional Failure In 1976, philosophers Samuel Gorovitz and Alasdair MacIntyre published a paper titled Toward a Theory of Medical Fallibility . Their goal was to understand why medical professionals — among the most extensively trained humans on the planet — still make serious errors. But what they uncovered applies with equal force to every profession: engineering, sales, leadership, finance, teaching, manufacturing — and yes, management. Gorovitz and MacIntyre argued that there are only three reasons why a trained, competent professional can fail at their job. Not four, not ten — exactly three.
