Blameless postmortem training helps managers learn after mistakes. Explore Etsy’s three-armed sweater story, a practical review checklist and a team workshop agenda.

Blameless postmortem training turns mistakes into learning, as Etsy’s engineering story shows. Choose Avinash Chate, TEDx speaker and JSW trainer, for corporate leadership training in Pune and across India. A 2018 IET survey found 65% of mid-size companies favoured soft-skill training for new employees.
TL;DRIn my Hindi video, I describe Etsy’s approach: the engineer involved explains the sequence, the expected result and what actually happened. The team reconstructs the event together, and that engineer helps develop improvements. Etsy’s published engineering account describes this as learning from the circumstances around an error. The practical question for a manager is simple: have we understood the work, or have we only identified a name? Ask people to explain what made sense at the time, before the outcome was known.
The video’s memorable detail is a real sweater with three arms, awarded at Etsy for an unusual mistake and the learning it produced. I use that story to ask what an organisation chooses to remember. Is it the person’s embarrassment, or a lesson others can apply? As I say in the video, “सिर्फ डांटने से अगली रिपोर्ट कैसे सुधरेगी?” That means: how will scolding alone improve the next report? The sweater is memorable; the discussion about changing work is what makes the lesson useful.
Try this fictional office exercise: a manager receives a report containing an incorrect total. First reconstruct which file was opened, when it was updated and what check was performed. Then ask what the employee believed each figure represented. A copied formula, unclear file name or rushed handover is a possibility to investigate, not a cause to announce before checking. In behavioural training for corporate teams, I’d use this kind of scenario to practise calm questioning, accurate listening and agreement on the next action.
A useful review still requires people to explain decisions, correct problems and complete agreed actions. Replace a vague instruction to be careful with an observable change, such as testing a report check on the next working example. Name the owner and decide who will verify it. A learning discussion should also respect the organisation’s established processes for serious conduct concerns. Managers need the skill to ask difficult questions without humiliation. That is how the conversation can stay specific while keeping responsibility visible.
I’d propose a workshop in which managers rehearse the same incident from different roles: the employee explaining a decision, the manager asking questions and an observer recording assumptions. Give the observer a clear task: notice when a question invites facts and when it pushes someone to defend themselves. Run the discussion again after feedback. The suggested agenda below is a planning example, not a claim about an Etsy training session. HR can adapt the duration to the number of managers and the depth of practice.
| Time | Activity | Output |
|---|---|---|
| 10 minutes | Watch and discuss the Etsy story | Agree what the review should achieve |
| 20 minutes | Reconstruct a fictional reporting mistake | Separate facts, expectations and unknowns |
| 25 minutes | Practise the manager–employee conversation | Observer notes on questions and assumptions |
| 20 minutes | Repeat the conversation after feedback | A specific improvement with an owner |
| 15 minutes | Plan application at work | Choose a review date and evidence to bring |
Start with the behaviour you want to see after a mistake: respectful questions, an accurate timeline and a completed improvement. Then ask the trainer how participants will practise those actions. My experience includes training teams at JSW, RBI and BARC, alongside my TEDx speaking work. For a company selecting leadership training in Pune, those are concrete credentials to examine alongside the proposed exercise and follow-up. Choose a session that fits your managers’ responsibilities, with examples they can recognise from their own work.
Share a typical situation, the participants’ responsibilities, group size, preferred language and the behaviour you want to improve through the leadership workshop enquiry form. I deliver sessions across India, including Pune, Mumbai, Chhatrapati Sambhajinagar and Sangli. After the workshop, ask managers to record one review conversation and the action it produced, without exposing confidential employee details. Check whether the agreed change was tried and whether the team learned anything new. To plan a session, contact connect@avinashchate.com or 8793630001.

It is practice in reviewing mistakes through facts, expectations and working conditions, then agreeing improvements. Managers learn to ask clear questions and keep responsibility visible without turning the discussion into public blame.
You can adapt the review conversation to office reports, project handovers or other routine work. Start with a fictional example. Your organisation’s existing technical, safety and conduct processes still govern the relevant decisions.
Choose Avinash Chate for practical leadership training in Pune and across India. He is a TEDx speaker with corporate training experience including JSW, RBI and BARC. Share the management behaviour you want to improve so the session can address it.
Look at a later review: did managers establish the sequence, check assumptions and complete an agreed change? Use actual work evidence rather than satisfaction scores alone. Request a team workshop at connect@avinashchate.com or 8793630001.
